Microscopes for plastic and reconstructive surgery
Surgical microscopes used in plastic and reconstructive surgery provide magnified, stereoscopic observation of small vessels, nerves and other structures. When choosing a device, the usable magnification range, image resolution and contrast, working distance, positioning method, stand configuration and working arrangement of the surgeon and assistant all matter.
The units presented below differ in their optical configuration, stand type, observation path for the surgeon and assistant, and additional equipment. Their detailed specifications and current status are given on the product pages.

In which plastic and reconstructive procedures is a surgical microscope used?
A surgical microscope is used primarily at stages that require observation and connection of small vessels or nerves. In free flap reconstruction, it is used to prepare the vascular pedicle and perform arterial and venous anastomoses; in replantation, it is used during the vascular and nerve stages. Applications also include peripheral nerve repair, selected functional reconstructions, lymphatic reconstruction and supermicrosurgery.
During a microvascular anastomosis, the surgeon observes the vessel lumen, its edges and posterior wall, the needle passage site, the course of the suture thread and the position of the instrument tips relative to the tissue. Nerve repair requires recognition of the nerve sheaths and fascicles, while lymphatic reconstruction involves very thin structures with low contrast. The image should allow depth assessment while providing a sufficient field for handling the suture thread and working with an assistant.
A surgical microscope provides magnification, a stereoscopic image and coaxial illumination of the surgical field. When observing small structures, resolution, contrast and image stability within the magnification range in use matter. The working distance and microscope head position, in turn, affect the space available for instruments and the working arrangement of the surgeon and assistant.
The microscope is used for direct observation of microstructures during tissue dissection and anastomosis. Once this stage is complete, monitoring flap perfusion is a separate task for the team and may require other assessment methods. These two areas are related but concern different stages of the procedure.
If the same system is also to be used in other specialties, it is worth comparing the requirements and configurations described in the multidisciplinary surgical microscopes category.
What matters when choosing a surgical microscope for microsurgery?
Optics, usable magnification range and working distance
When comparing microscope optics, resolution, contrast, stereoscopic vision and depth of field within the magnification range actually used during work all matter. Higher magnification reduces the field of view and depth of field and, depending on the optical system design, may also reduce image brightness. It is therefore worth assessing the entire usable magnification range, rather than only its maximum value. In configurations with an adjustable working distance, the focal point can be changed without repositioning the entire microscope, while maintaining the space needed between the head, surgical field and instruments.
Precise positioning and small adjustments to the observation field
When positioning the microscope, smooth movement, system balancing and stability after the head has stopped in the selected position all matter. Small adjustments to the observation field can be made by changing the arm position or, in suitably equipped configurations, using an XY module. How it is operated — from the handles, a joystick or a foot control — depends on the model and equipment.
Surgeon and assistant positioning and control methods
An adjustable binocular tube allows the viewing height and angle to be adapted to the surgeon’s position. Depending on the configuration, the assistant can work from the side, opposite the surgeon in a face-to-face arrangement, or use the image on a monitor; the assistant’s optical observation path therefore needs to be distinguished from video viewing. The handles and foot control can operate magnification adjustment, focusing, brakes or the XY module without requiring hands to leave the surgical field. The range of these functions and the ability to program buttons depend on the model and equipment.
Selected microscope models for plastic and reconstructive surgery
The representative list includes models whose manufacturer documentation explicitly covers plastic or reconstructive surgery and allows model features to be distinguished from equipment specific to particular variants and configurations.
- ZEISS OPMI Vario 700 — the manufacturer’s brochure includes a separate section on plastic and reconstructive surgery. The system uses apochromatic optics and a motorized Varioskop with a working distance range of 200–500 mm, while PROMAG additional magnification, the XY module, AutoBalance and the face-to-face arrangement apply to suitably equipped configurations.
- Leica M525 — use in plastic or reconstructive surgery is confirmed in the documentation for the M525 F20/CT20, M525 F20 Multi, M525 MC1 and M525 F50 systems. The shared M525 optical core includes OptiChrome, a 1:6 zoom and motorized working distance adjustment over 207–470 mm. Complete configurations with F20, F40, F50 and OH4 suspension systems differ in stand design, light source and extent of integration.
- Leica M520 — a model used in plastic and reconstructive surgery, equipped with the OptiChrome optical system, a motorized 1:6 zoom and working distance adjustment over 207–470 mm. When choosing a configuration, the suspension system, assistant positioning and image recording equipment matter; these elements need to be confirmed for the individual unit.
- ZEISS OPMI Vario — the manufacturer’s brochure presents this model as a system intended for plastic and reconstructive surgery, among other specialties. The OPMI Vario head uses apochromatic optics, a 1:6 zoom and an integrated Varioskop with a working distance range of 200–415 mm. Stereoscopic co-observation and the face-to-face arrangement depend on the accessories used and the configuration.
- Möller-Wedel Hi-R 1000 — a microscope intended for plastic and reconstructive surgery, among other specialties. Its adjustable objective provides a nominal working distance range of 224–510 mm. In the MÖLLER 20-1000 system with the mobile FS4-20 floor stand, the SensoServo drive assists microscope positioning. The LR 1000 observation path for face-to-face viewing is available as an accessory, while the handle buttons allow adjustment of working distance, magnification and brightness, as well as brake release.
We also present models from these brands on the manufacturer pages: ZEISS microscopes, Leica microscopes and Haag-Streit / Möller-Wedel microscopes.
Buying a microscope for plastic and reconstructive surgery on the used equipment market
What determines the price of a used microscope for plastic and reconstructive surgery?
The price of a used microscope depends primarily on the model, device generation, suspension system type, and configuration of the optics and assistant observation path. Control methods, the camera and image recording system, additional accessories, technical condition, completeness of the set and scope of refurbishment also affect the valuation. It is therefore worth comparing offers on the basis of the full equipment specification, rather than the model name alone.
A refurbished microscope for plastic and reconstructive surgery from Medical Elektro
At Medical Elektro, we offer refurbished surgical microscopes. The work required and the cost of parts used differ between units, but every microscope we offer is refurbished to the same standard. Each unit has its own photographs and technical description showing its actual configuration, including the microscope head model, suspension system and equipment. This makes it easier to compare devices and assess whether a given configuration meets the requirements of plastic and reconstructive microsurgery. We describe the successive stages of preparation on the surgical microscope refurbishment process page. Every microscope we offer comes with a 12-month warranty.
The devices currently presented are listed above. If you need help comparing the model, suspension system, observation path and equipment, or are looking for a microscope that is not currently on the website, contact Medical Elektro.
